My practice accepts the following insurance:

Some of these plans MAY be managed under American Specialty Health, which I am in In-Network Provider. I am NOT a provider with Aetna, Kaiser or Molina plans.

As a courtesy, your insurance benefits can be verified prior to your initial appointment. However, the verification of benefits is NOT a guarantee of payment. Ultimately, payments are the responsibility of the patient. Patients should be familiar with their individual medical insurance plan coverage and benefit details.

Please be advised that insurance ONLY COVERS ACUPUNCTURE. Adjunctive modalities such as Cupping, Gua Sha or Massage are non-covered services, but are available as Self-Pay service.

Office Visit Fees such as Co-Pay and Deductible amounts are due at the Time of Service.
